Fall sport physicals, concussion testing & athletic fees are due to the athletic office -Thursday, JULY 17TH PHYSICALS – - Print, complete & submit the PIAA physical form (6 pages, dated 7/17/24), along with St. Luke’s HIPPA & Consent to Treat forms (2 pages) on the BMSD website to the athletic office by 7/17/24.
- Please be sure to take the PIAA packet to the scheduled appointment (private or school).
- Physicals may not be dated prior to MAY 1, 2025.
CONCUSSION TESTING – - Mandatory for all athletes (grades 7-12) participating in a school sponsored sport.
- Testing must be completed every school year, students may not participate without it in any sport.
- Concussion testing is completed at home – no forms need to be printed.
- Please note the test must be taken on a computer and NOT ON a PHONE or IPAD. Instructions can be found on the athletic page of the Blue Mountain website under quick links.
ATHLETIC ACTIVITY FEE – - The $60 athletic fee (per sport) must be paid by July 17th in order to participate in a FALL sport.
- Acceptable methods of payment: money order or online payment.
